Proyecto Conexiones:

Federally supported program, for
clients 18 and older and that are Philadelphia resident, enhances and expands
APM’s substance abuse treatment services, by reaching out and attracting
community members and Latino ex-offenders into our substance abuse treatment and
HIV prevention activities. Program services include case management referral
services to APM and other community agencies, referrals for medical and
psychiatric evaluation, and close coordination with APM’s
Proyecto Nueva Vida
for drug and alcohol individual and group services.

Referrals to our program are
accepted from individuals, social service providers, agencies, hospitals and the
legal system. We do not discriminate on the basis of ethnicity, sex, sexual
preference, national origin, or ability to pay.

Hours of Service: Monday, Wednesday, and Friday, 8:30 AM -
5:00 PM
Tuesday and Thursday,
Proyecto Nueva Vida is open between 8:30 AM and 6:00 PM.
